A casual enrolment is not a full enrolment.
This form is just so we can collect the necessary details to see you for a one-off or occasional visits while you remain enrolled with your usual GP elsewhere or if you’re not eligible to enrol.
What casual enrolment means
- It is not a full enrolment.
You are being seen as a casual (non-enrolled) patient, not registered with a provider at Taupō Medical Centre. - Standard visit fees apply.
Casual visits are not subsidised, so you will not receive the lower enrolled-patient rates. Extra charges apply and payment is required at the time of your visit. - Your current GP stays the same.
Filling out this form will not unenrol you from your existing GP or practice. You remain enrolled where you are now.
