TAUPO MEDICAL CENTRE-TEAM CARE APPROACH-PODS
Taupo Medical Centre became what is known as a Health Care Home in 2016.
A Health Care Home is a primary care design to promote improved patient care and also to provide a sustainable system.
There have been a number of improvements and developments along this journey.
These include renovating the building, the ‘my indici’ patient portal, options for virtual consultations (video and phone), e-prescribing and a new patient management system. We have also added Health Improvement Practitioners, a clinical pharmacist, Medical Centre Assistants and Nurse Practitioners to the TMC Team.
The Health Care Home model is focused around the patient – Taupo Medical Centre now provide a ‘TEAM’ (which includes you, the patient) to manage your health.
These teams are called ‘PODS’. They consist of 3 Doctors, a Nurse Practitioner and a Medical Centre Assistant. They are supported by the Health Improvement Practitioners, the Clinical Pharmacist and the Nurses.
The aim is to provide better healthcare collectively.
We aim to improve continuity of care overtime by helping you develop a relationship with all members of the team, any of whom can provide care to you based on your needs.
